PARTICIPANT REGISTRATION FORM
RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT AND PARENTAL CONSENT
Please use the following waiver when you have any participant that is a minor
In consideration of participating in activities at Flying High Gymnastics, Inc, I represent that I understand the nature of these activities and that I am qualified, in good health, and in proper physical condition to participate in such activities. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that these activities involve risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost and damages I incur as a result of my participation in these activities.
I hereby release and discharge Flying High Gymnastics, Inc., its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and owners and lessors of premises on which the Activity takes place (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releases" or otherwise, including negligent rescue operations and further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releases, I will indemnify, save and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
PARENTAL CONSENT
AND I, the minor's parent and or legal guardian, understand the nature of the above referenced activities and the Minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby Release and discharge, AGREE TO INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses or damages on the minor's account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss, liability, damage or cost any Releasees may incur as the result of any such claim.
Flying High also has permission to use my child's picture in marketing publications without compensation to me.

SANTA OPEN GYM

LITTLE TYKES TIME
Enjoy quality time with your little tyke
Offered Wed/Thurs 10-11, Friday 1-2pm
mid August thru mid June. Summer LTT hours: Wed 10-11am, Thursdays 1-2pm
for just $5! Watch them have fun with our mini inflatable, slides, tunnels, trampolines, foam pits, swinging ropes, mini bars, and much much more!
SCHOOLS OUT
What do we do with the kids? Plan your play dates with us!
*Schedule your Early Dismissal Days and School Days Off with us and we’ll plan a full day or half day full of fun, friends & fitness based on the student’s school calendar.
PIZZA FUNdraising NIGHTS OR LUNCHES!
We put the “FUN” in Fundraising!
*Whether you’re a school, community organization, church or group that wants to raise funds you won’t believe how easy it is! We will work with you to reach your fundraising goals.
PIZZA FUN NIGHT
Come party with us the first Friday of every month from 7-10PM. Kids, ages 5 & up will enjoy Inflatable Open Gym time, games, pizza and lemonade. The Cosmic Café will be open during all Pizza Fun Nights.
SCOUT PARTIES
Calling all Leaders! Bring the Troop, Pack or Den!
*Boy Scouts and Girls Scouts can earn patches, belt loops and pins with our custom designed parties. Themes include physical fitness, gymnastics, sports and cheer. A minimum of 10 children per party is required.
OPEN GYM
Two hours of continuous fun for all ages!
Inflatable Open Gym every Saturday 12-2pm
mid August thru mid June. Summer O.O hours are Wednesday 11-1, Saturdays 12-2
*Non-structured, supervised meeting time for anyone and everyone who wants to burn some energy. Open gyms are held every Saturday from September through May from 12:00-2:00.
Summer open gym is every Wednesday from 11:00-1:00.
Adult Open Gym
Tuesdays 8-9pm September thru early June. For high school age and older. Anyone under 18 years must have a waiver signed by a guardian. Adults have the opportunity to brush up on some gymnastics skills or use our equipment to stay fit!
WINTER SHOWCASE
Families & Friends please join us for an exciting weekend event held in February. All class levels are invited to participate. Every child has their moment to shine! Team girls will demonstrate progression of skills. Each child is awarded a trophy for their individual abilities. Everybody wins! Click here for more info!
FIELD TRIPS/BONDING TRIPS
Different destinations every time! We will plan a fun filled day based on the holiday and include open gym and arts & crafts.
HOME SCHOOLS
Come join us for an afternoon of physical fitness! Our gym time can include games, obstacle courses or inflatable fun! We will meet your needs.
TRAMPOLINE CAMP
Kids will learn basic tricks and safety tips on the trampoline
BOYS SPORTS READINESS CLINIC
This will focus on strength, agility and teamwork as well as specific skills needed for todays most popular sports.
FULL DAY SUMMER CAMP
Boys and girls will have a full day of fun participating in group activities such as relay races, obstacle courses, open gym, inflatables, awesome field trips, nature walks and crafts all while learning life lessons.
SCHOOL BREAK CAMPS
Keep the kids active and learning with fun activities during school breaks. Whether it’s flipping, dancing, sports or cheer we have many camps to choose from.
HALLOWEEN SLEEPOVER
Kids will have a spooktacular time with treats, games and prizes. Costumes are optional.

I would like to extend a HUGE thank you to Flying High on behalf of the families at Forest Road School.
When our playground committee was trying to come up with fund raising ideas, I immediately thought of Flying High.
My children have been members at Flying High since they were 3. Since that time, we've enjoyed many Winter Tumbles, Open Gyms, birthday parties and countless Pizza Fun Nights. With so many wonderful things happening and a new facility, I knew Flying High would be a great place to host a fundraiser. We came up with the Forest Road/Flying High Pizza Fun Night.
As the liaison between our committee, the school, and Flying High, I couldn't be more pleased with the success of this fundraiser. We scheduled 3 events and have raised over $2700. The buzz around school has been great! All the kids (Pre K-6) want to attend these events. Parents have been thrilled with not only an evening out, but more importantly, the excellent supervision and activities made available at Flying High. It's been such a success, we are hoping to schedule more in the spring! Thanks again, Flying High!
